Covid-19 Vaccines in Florida Moving to Next Phase
Starting today, those under 65 deemed by a physician to be “extremely vulnerable to COVID-19” can obtain a COVID vaccine in Florida.
To do so, you must present the attached Florida Department of Health form
to a medical provider for their signature:
